There are 2 kinds of neck wrinkles, and almost every guide only addresses one. The first is the aging and expression kind: collagen declines, sun damage accumulates, and looking down at a phone etches horizontal lines across the neck. Creams, retinoids, sunscreen and injectables target these. The second kind is mechanical. Plastic-surgery research calls them sleep wrinkles, and they form from compression, tension and shear when your face and neck are pressed against a sleep surface, not from muscle movement. The distinction is not academic: because these lines are not caused by muscle contraction, botulinum toxin cannot eliminate them. And you spend roughly 65% of the night on your side, which is exactly the position that folds the neck and presses the décolleté. What causes neck wrinkles?

Neck skin is a harder place to age well than the face. It is thinner, it has fewer oil glands so it dries out faster, and per the Cleveland Clinic it has fewer hair follicles, which makes it "more prone to showing signs of aging." Most people also under-treat it, stopping their skincare at the jawline. Layered on top of that are two separate wrinkle-forming processes.

What are sleep creases, and how are they different?

In 2016, a review in the Aesthetic Surgery Journal treated sleep wrinkles as a distinct category rather than a variation on ordinary aging. Its finding was direct: sleep wrinkles "form in response to distortion created when the face is pressed against any sleep surface," with compression, tension and shear forces acting on the tissue. They differ from expression wrinkles in mechanism of origin, in location, and in the direction they run.

Two consequences follow. First, the standard tools miss them: neurotoxins work by relaxing a contracting muscle, and there is no contracting muscle here. Second, the exposure is enormous and repetitive. The lateral position accounts for about 65% of sleep time, so over years the same fold is pressed into the same place for thousands of hours, on skin that recovers more slowly each year. The same one-sided mechanics show up on the face, which is why smile lines are often deeper on one side.

Worth being precise here, because this is where the category usually oversells. In younger, elastic skin a sleep crease is temporary and gone by mid-morning. The concern the literature raises is cumulative: repeated distortion over decades, on skin that springs back less each year. That is a reason to reduce the load, not a reason to panic about one morning's lines.

Why the neck and chest crease more than anywhere else

Geometry and anatomy compound each other. When you turn onto your side, the head tips toward the shoulder and the neck skin gathers into a fold that stays folded for hours under the weight of your head. At the same time the chest rotates inward, pressing one breast against the other and creasing the décolleté along vertical lines. Neither area gets the relief the face gets from shifting position, because the fold comes from the posture itself, not just contact.

Then the tissue works against you. Neck and chest skin is thinner, has fewer oil glands and less structural support, so it folds more readily and recovers more slowly. This is why "chest wrinkles from side sleeping" is a specific complaint rather than a general aging one.

What actually helps, in order of evidence

Ranked honestly, strongest evidence first.

  1. Sleep on your back. This is the intervention the research actually points to. Dr. Anson's summary is blunt: "If you can stay on your back, that's ideal." It removes the folding entirely, and it is also the hardest habit here to keep.
  2. Treat the aging kind properly. Daily broad-spectrum SPF 30 or higher, a lower-strength retinoid a few evenings a week, and antioxidants such as vitamin C. Take the routine you already use past your jawline; most people stop at the chin.
  3. Support the neck rather than folding it. A pillow that keeps your head roughly level with your spine reduces how sharply the neck gathers. Piling up two pillows increases the fold.
  4. Reduce the forces at the surface. Compression comes from your own head weight and you cannot remove it. Shear, the sideways dragging force, depends on how much the surface grips your skin, and that is a property of the fabric.
  5. Keep the skin hydrated and the surface dry. Well-hydrated skin recovers its shape faster, and a surface that holds sweat against the neck all night works against that.

Does your pillowcase affect neck wrinkles?

It affects one of the three forces, and it is worth being exact about which. The 2016 review examined sleeping position, not fabric, and did not test or endorse any pillowcase material. What it established is that shear is one of the forces distorting the tissue. Shear is friction: the sideways drag as your skin moves against the surface while you settle and turn. A rougher surface grips more and drags more.

Friction is measurable, which means this is one of the few parts of the wrinkle conversation you can actually check a number on. Cotton's surface roughness measures about 0.25 µm. Can you get rid of neck wrinkles?

You can soften them, and how much depends on which kind they are. Aging and expression lines respond to sunscreen, retinoids, antioxidants and, for deeper cases, lasers, microneedling or fillers. Sleep creases respond to changing the mechanics: back sleeping, better neck support, and less drag at the surface. Established lines rarely disappear completely, so prevention returns far more than correction.

Does sleeping on your side cause neck wrinkles?

It contributes to one of the two kinds. Side sleeping folds the neck and presses it against the sleep surface, producing the compression, tension and shear the Aesthetic Surgery Journal review identified as the origin of sleep wrinkles. With the lateral position taking roughly 65% of the night, the exposure is substantial. It is not the only cause, and sun damage remains the larger driver of the aging kind.

Does Botox work on neck wrinkles?

For some of them. Neurotoxins relax the platysma muscle and can soften the vertical bands and horizontal lines driven by muscle activity. What is tech neck?

How do you prevent chest wrinkles from side sleeping?

Reduce the fold and the drag. Back sleeping removes the compression altogether.
